Pending the upcoming release of a fully digital travel platform by Mauritian Authorities, you may opt to use the Mauritius All-In-One Travel Form as an alternative to manual filling of the Immigration Disembarkation Card and four other paper forms required by the Health Authorities.

All you need to do is fill out the necessary information on the Mauritius All-In-One Travel Form and select Send me my PDF.

Within a minute or two, you will receive an e-mail from noreply@mauritiustravelform.com with a completed PDF document comprising four different forms in a format identical or similar to the existing paper forms:

  1. Disembarkation Card (Page 1 – Upper section) 
  2. Health Declaration Form (Page 1 – Lower section) 
  3. Health Declaration Form, Part II (Page 3) 
  4. COVID 19 Laboratory Request Form (Page 5)

We have purposefully included two blank pages to avoid double sided printing. Kindly print each page on a separate sheet, split/tear the first page into two parts as per indicated marking and duly sign the relevant pre-filled forms.  

Present the forms, along with any other mandatory travel documents, to the Border Control and Health Authorities on arrival in Mauritius - and potentially to your airline if requested before your flight.  The last form (COVID 19 Laboratory Request Form) is only necessary if you are required to undergo a PCR test at the Airport. You may note that none of your personal data has been retained on our servers and are thus no longer available for any further use.  Should you wish to amend any personal information on your form, if the Mauritius All-In-One Travel Form is still open on your browser, you may still do so and click Send me my PDF again.  If not, you must fill the form anew.

 

In the event that you cannot complete this form online, you still have the option to fill out the following paper forms by hand. 

Visit the website of the Ministry of Health and Wellness to download the Passenger Locator Form and Self-declaration form. 
 
If you are fully vaccinated as per local sanitary requirements and would be residing in a place other than an approved hotel/resort, you will have to undergo a PCR Test at the airport. For this purpose, you will be requested to submit the duly signed COVID 19 Laboratory Request Form.
 
You will be given the Immigration Disembarkation Card and Health Declaration Form on board of your flight.  

 

This form can be filled within 72 hours of your departure once you have received your negative PCR Test result*.  You will need to print and sign the PDF document received upon submission of the form.

* In order to avoid last minute stress, you may also fill the form before you obtain your PCR Test results - provided you input the Date of the PCR Test, lab name and country correctly.  Remember, your form submission is only valid once printed and duly signed by hand.

When you click the Send me my PDF button, you should briefly see the message as per screenshot below:

If you do not see the above message, please click the Send me my PDF button again then the Confirm button.

Within a minute or two after seeing this message, you will receive an e-mail from noreply@mauritiustravelform.com with a completed PDF document.

In case you do not receive the email, try the following:

  • check if the email address input on the form is correct
  • check your spam folder
  • make sure your mailbox is not full/over quota
  • try with an email address from a different e-mail provider.  The automated email may have been rejected by your email provider.  

If none of the above works and you are in a hurry, consider downloading the forms and filling them by hand. 

You may wish to note that the Date of Departure from Origin is used to verify both your pre-departure PCR Test status and your vaccination status.  Which date should it be?

Example 1

If you are taking a direct flight from London Heathrow to Mauritius, your Date of Departure from Origin would be the date of departure from London Heathrow.

Example 2

If you are flying to Mauritius from London Heathrow via Dubai (where you are only in transit), your Date of Departure of origin would be the date of departure from London Heathrow.

Example 3

If you are flying to Mauritius from London Heathrow via Dubai (but exiting the Dubai Airport for a few hours or days), your Date of Departure of origin would be the date of departure from Dubai.

 

 

The Last port of embarkation refers to the port of embarkation for the flight to Mauritius.

Example 1

If you are taking a direct flight from London Heathrow to Mauritius, the Last port of embarkation would be London Heathrow.

Example 2

If you are flying to Mauritius from London Heathrow via Dubai, the Last port of embarkation would be Dubai.

 

If you are fully vaccinated, please try again as follows:

 

- Date of arrival and date of departure from origin must be input

- Question Are you a Resident? must be answered

- The vaccination certificate checkbox must be ticked

- The State Approved/Registered Hotel Voucher checkbox must be ticked if you would be staying in an approved/registered hotel

- The State Approved/Registered Hotel Voucher checkbox must be unticked if you would be staying at an accommodation other than an approved/registered hotel

One of the most common reasons why this error occurs is that of an outdated browser version. ReCaptcha will actively look at the browser version before allowing you access.  In this case, the solution is to update your browser to the latest version (see minimum browser requirements).
 
The second solution is to clear your browser cache and cookies.
 
If your issue is still not solved, you may try the following:
  • VPN service is not playing well with reCaptcha – Try turning off VPN.
  • Check that JavaScript is enabled in your browser
  • Try disabling plugins that might conflict with reCAPTCHA
  • Try with another device
 
Your problem should be resolved.  If it is not the case, you are left with the option to fill out the paper forms by hand as follows:

If you are filling the all-in-one travel form, you can select your hotel from the list of state approved/registered hotels displayed.   

You may also check the list of licensed hotels from the website of the Mauritian Tourism Authority.  Note: This list also includes quarantine hotels.  If you want to make sure you do not pick a quarantine hotel, click here

Sometimes, your hotel name may be written differently in the list.  For example,  "Mauricia Beachcomber Resort & Spa" may be displayed as "Le Mauricia Hotel".  To be sure, you may google the hotel name and address or check with your hotel.

You need to print and duly sign the forms by hand for submission to the Authorities on arrival in Mauritius.

  • If you test positive for COVID-19 on your day 0 PCR test, you will be contacted by the authorities by phone within 6-12 hours
  • If you test positive for COVID-19 and are asymptomatic, you will have to isolate for 10 days within your accommodation
  • If you test positive for COVID-19 and are experiencing worrying clinical symptoms, you must call the Ministry of Health and Wellness on 8924 to be assessed.You may be hospitalised in a public or private healthcare facility of your choice
  • Your travelling companions must also isolate for seven days

If you will be staying in an accomodation other than a state approved/registered hotel, e.g. your own residence, an AirBnB villa, guest house, etc., you would be required to take a PCR test at the airport.

You will be guided to the "Swab Area" for the PCR test after passing Border Control and Health Counters.  No need to worry as there is no way you could miss it.

You may collect your luggage and leave the airport immediately after your test. 

Your test results will be available within 6 to 12 hours.  However, the Health Authorities will only contact you if your result is positive.

Regardless of the airline or vaccination status, travellers including children and infants must all undergo a PCR test 72 hours prior to their departure for Mauritius. An oropharyngeal swab can be collected for children up to 6 years of age.  A negative PCR test result is required to be allowed to travel.

The following rules will also apply for travellers including children and infants.

  • If you are staying at a hotel, antigen tests will be carried out at the hotel on arrival (day 0) and on day 5.
  • If you are staying at other accommodation on the island, or returning to your home, you will be required to take a PCR test on arrival at the airport (day 0).   A self-administered antigen test will also be required on day 5 of your stay.

 

You have more than 4 family members flying with you.   You can use the Travel Companions (Non-family) section to add two more members of your family.  You'll strike off "Non-family" on the printed form.  
 
If you have more than 6 family members, you may add them by hand on the verso of the printed form.
